NHS, Private or Membership? A Straight Guide for Thurrock Patients

It's the question we're asked more than any other at our practice in Civic Square: what's the real difference between NHS and private dentistry — and is it worth paying?

Most patients think it's a straight choice between staying on the NHS or paying full private fees for everything. It isn't. There's a third option in between, and for most of the people who ask us, it's the one that actually fits.

Our membership starts from £22.80 a month — around 75p a day. That covers two oral health examinations and two gum assessments with professional cleaning every year, discounted routine treatment, priority same-day urgent care, and worldwide dental emergency assistance. Here's what that means in practice, and how it compares with the alternatives.

The straightforward case for it is arithmetic. Paid separately at our published rates, two examinations (from £69 each) and two hygiene appointments (from £79 each) come to £296 a year. Membership is £273.60 — and that's before the treatment discount, the priority access or the emergency cover.

But the stronger case isn't the £22 you save on the routine appointments. It's what regular attendance does to your dentistry over ten or twenty years.

Why being seen regularly changes the numbers

Almost everything we treat starts small. Decay begins as a spot on an X-ray before it's a hole. Gum disease starts as bleeding when you brush, years before anything loosens. Caught early, these are quick, cheap and comfortable to deal with. Left alone, the same problems become root canal treatments, crowns and extractions.

The gum assessments are the part people underestimate. Gum disease is usually painless until it's advanced, which is exactly why it's the leading cause of adult tooth loss in this country. A professional clean twice a year, with someone actually measuring and recording what your gums are doing, is the single most effective thing most patients can do to keep their own teeth.

Same-day urgent care matters for the same reason. A cracked tooth or a lost filling seen this afternoon is usually a simple repair. The same problem two weeks later can be a root canal.

That's the honest logic of membership: it isn't a cheaper way to buy treatment, it's a way of needing less of it.

What private care gives you

Whether you're a member or paying as you go, private care differs from NHS care in scope rather than standard. The dentists are the same dentists — everyone here is registered with the General Dental Council and works to the same professional standards regardless of how an appointment is funded. What changes is what's on the menu.

• You choose your dentist, and you keep them. Over years rather than visits, a dentist who knows your mouth and your history makes a real difference.

• Longer appointments, so there's room to talk through options and ask questions.

• A wider choice of materials — zirconia and Emax crowns, cobalt chrome and flexible dentures, premium composites. These often come down to how natural something looks and how long it lasts.

• Faster onward referral, and a say in where you go. An NHS referral for advanced treatment means joining a hospital or community waiting list, with no choice over who you see. Privately we can refer you quickly, and you choose where.

Treatments the NHS doesn't cover at all, including:

• Dental implants (except exceptional cases, usually via hospital after trauma or cancer treatment)

• Teeth whitening — home kits from £375, Enlighten from £650

• Veneers and composite bonding provided for appearance

• Invisalign and adult orthodontics — NHS orthodontics is for under-18s meeting strict clinical criteria

• Hygiene appointments beyond clinically necessary scaling

• Sports gumshields

Why NHS dentistry works the way it does

It's worth understanding how the system is funded, because it explains a great deal — including things that are often blamed on individual practices.

NHS dental practices are paid for courses of treatment delivered, not for keeping patients healthy. It is a system built around repairing problems, and it does that job at a price nothing in private dentistry can match. What it isn't built to fund is the slow, unglamorous work that stops problems developing in the first place — the regular monitoring and gum care that means you need less dentistry in ten years' time.

That funding model is also why NHS dentistry has become so much harder to access. Fewer dentists are taking on NHS work each year, and most practices in England are not currently accepting new adult NHS patients.

Membership is built the opposite way round. You're seen regularly whether or not anything is wrong, and the gum care is included rather than being something that has to be justified as clinically necessary before it can happen.

Our NHS position, stated plainly

We think you're entitled to know exactly where we stand before you ring us, so here it is.

• We are not currently accepting new adult NHS patients. We'd rather say that on our website than have you find out after a phone call.

• If you're already an NHS patient with us, you stay an NHS patient for as long as you want to. Nothing changes and we're not asking you to move.

• We do provide NHS care for children. NHS dental treatment is free for under-18s, and we're glad to see them.

• We do provide NHS urgent and unscheduled care. If you're in pain, ring us — every NHS practice is now contractually required to keep a minimum volume of urgent appointments available, set at 8.2% of contract value from April 2026. These appointments deal with the immediate problem rather than providing a full course of treatment, and they're subject to availability.

If you're an adult looking for a routine NHS dentist, the NHS "find a dentist" service at nhs.uk lists practices in Thurrock and Essex and shows which are taking new patients. We'd genuinely rather point you there than have you go without care.

For context, NHS charges from April 2026 are £27.90 for Band 1, £76.60 for Band 2 and £332.10 for Band 3, with one charge covering a whole course of treatment however many items it involves. NHS treatment is free if you're under 18, under 19 in full-time education, pregnant or within 12 months of giving birth, or receiving certain means-tested benefits.

One further change from April 2026 worth knowing: NHS check-up intervals are now set by individual risk assessment following NICE guidance — anywhere from three months to two years — rather than everyone being booked in every six months by default. For a genuinely low-risk patient a longer gap is clinically appropriate and evidence-based, but it does mean some patients are seen less often than they're used to.

Which is right for you?

Membership suits you if you'd rather be seen regularly than wait until something hurts, you want the same dentist each time, you value being seen the same day in an emergency, and a fixed monthly cost is easier to manage than an occasional lump sum.

Two points of clarity, because we'd rather you knew them now than later.

Membership doesn't sit alongside NHS care — it replaces it. If you're currently an NHS patient with us and you join, you become a private patient rather than an NHS one. Nobody should sign up thinking they're simply adding extras to their NHS treatment, and we'll always say so before you commit to anything.

Paying privately as you go suits you if you want a treatment the NHS doesn't cover, or a wider choice of options and materials for a specific piece of work, without committing to a monthly plan.

Routine NHS care may suit you better if you're exempt from charges or you need a significant amount of treatment — in which case, as above, the NHS "find a dentist" service will show you which practices in Thurrock and Essex are currently taking on adult patients.

Families often split it: children seen on the NHS here, parents on membership. If you'd rather keep NHS care but want better gum health, seeing our hygienist privately from £79 a session is a common arrangement and doesn't affect your NHS treatment at all.
